Check out these resources designed to integrate the 5-2-1-0 habits into your daily routine. Whether you are a school, business, grocery store, or other entity, our resources are available for your use. Please feel free to share these with your colleagues, friends, and family. 5210 Flyers

General Flyer: Join the challenge for a healthier lifestyle. Share the 5210 General Flyer in your community to inspire friends, family, and colleagues to take on the challenge of implementing the 5-2-1-0 habits.

Business Flyer: Promote the 5210 Challenge at your business or place of work. Get the coworkers and suitemates involved in establishing some healthy habits.

Grocery Store Flyer: Promote the 5210 Challenge in your store. Help community members and staff make healthy choices by sharing this resource.

School Flyer: Promote the 5210 Challenge in your classroom. Get your students and fellow teachers involved.

Community Member Flyer: Share the 5210 Challenge with others in your community. Whether you are a grocery store, church, visitor’s center, or other location in your town, feel free to use this flyer to promote the challenge.

Samaritan Flyer: Promote the 5210 Challenge in your clinic, office or hospital site. Engage clients, patients, or visitors with this healthy activity that can be used to make positive & fun choices.

SamFit Flyer: Encourage SamFit members to take part in the 5210 Challenge. There is no better spot than the gym to get your 5210 physical activity in for the day!

5210 Trackers

Community Member Trackers: These trackers can be shared with community members at any location. Participants can track their progress on this paper tracker, but will ultimately need to enter their progress through their account on the website in order to be entered in for the chance to win a prize.

Library Trackers: Share these trackers with the general public at your local library. These trackers are to be distributed by the library and then collected at the end of the month by the library. Libraries can work directly with the 5210 coordinators to submit the trackers so that participants have the chance to win a prize.

Pre-K – 2nd Grade Trackers: These trackers are designed to be folded like a brochure. The cover explains the 5210 Challenge and includes the tracker on the inside. The tracker is a simple format where individuals can circle icons corresponding to their daily 5210 habits. These are the trackers to be used in the Pre-K – 2nd Grade 5210 School Challenge. Teachers will submit these at the end of the challenge via the Teacher Portal.

3rd – 5th Grade Trackers: These trackers are designed to be folded like a brochure. The cover explains the 5210 Challenge and includes the tracker on the inside. The tracker is a simple format where individuals can circle icons corresponding to their daily 5210 habits. These are the trackers to be used in the 3rd – 5th Grade 5210 School Challenge. Teachers will submit these at the end of the challenge via the Teacher Portal.

6th – 12th Grade Trackers: These trackers are optional for students to use in the 6th – 12th Grade 5210 School Challenge. Students must join their team via the website to enter in their progress. These trackers can be used by students and teachers who would like to have paper copies to keep track of their progress throughout the challenge.

1 Week Trackers: Take the challenge one week at a time with this visual tracker. This tracker can be used by all but it best for our older adults and Pre-K-2nd graders. Individuals can track their healthy habits while focusing in on just one week.

5210 Bulletin

Bulletin: Promote the 5210 in your church bulletin, soup kitchen or resource fair. These smaller 5210 flyers are perfect to use as inserts in other packets.
